Many of the Behavioral Forms have been revised to remove
duplicative information that was requested in the application and the
protocol. The protocol templates were also re-formatted so that information
is entered into a table. We did not make substantive changes in any of the
questions or information requested. We hope that the changes will make
completion easier. Please make sure you are using the most current forms by
going to the HRPO website instead of using forms saved to your desktop!
The Behavioral forms may be used by
investigators who are conducting minimal risk
social-behavioral-educational research. See the HRPO
