categories of research exempt from human subject regulations
(45 CFR 46.101(B)) and must meet ethical standards.

Application Requirements
The following materials are required of all new minimal
risk applications. Any applications that do not meet the
following criteria will be sent back to the PI without being
reviewed.
- Two (2) complete copies.
- Completed forms including all
signatures, spell checked, and typed

applicable, please submit:

Consent form, information sheet, brochure, and/or letter, script for verbal
consent

Grant application and budget with
confidential salary information and total dollar amounts blocked out

Recruitment materials (e.g., Questionnaires, flyers, advertisements,
telephone script, letters, etc.)
