Minimal risk means that the probability and
magnitude of [physical or psychological] harm or discomfort
anticipated in the research are not greater in and of themselves
than those ordinarily encountered in daily life or during the
performance of routine physical or psychological examinations or
tests [of healthy persons].

Application Requirements
The following materials are required of all continuing
review minimal risk applications. Any applications that do
not meet the following criteria will be sent back to the PI
without being reviewed.
- Two (2) complete copies.
- Completed forms including all
signatures, spell checked, and typed

applicable, please submit:

Consent form (unstamped), information sheet, brochure, and/or letter, script
for verbal consent

Form K1 – Application for a Waiver of Consent

Any currently revised materials (including protocol, grant, etc.)
-
Cumulative SAE report (Form 6 or electronic
submission report)

Most recent progress report for all Grant Agency sponsored research
-
Data Monitoring Report
-
Publication(s) to date resulting from this
research that were not previously submitted.
