New Submission - Biomed Expedited (Min Risk)

 

Minimal risk means that the probability and magnitude of [physical or psychological] harm or discomfort anticipated in the research are not greater in and of themselves than those ordinarily encountered in daily life or during the performance of routine physical or psychological examinations or tests [of healthy persons].

If you are not sure whether your study involves minimal or greater than minimal risk, please call the HRPO office at 633-7400.

Application Requirements
The following materials are required of all new minimal risk applications.  Any applications that do not meet the following criteria will be sent back to the PI without being reviewed.

  • Two (2) complete copies.
  • Completed forms including all signatures, spell checked, and typed
  • Typed study protocol

If applicable, please submit:

  • Consent form, information sheet, brochure, and/or letter, script for verbal consent
  • Entire Grant application (except for appendix pages) and budget with confidential salary information and total dollar amounts blocked out
  • Recruitment materials (e.g., Questionnaires, flyers, advertisements, telephone script, letters, etc.)
