The most current versions of HRPO forms are listed
and can be downloaded directly to your computer.

The forms are available only in Microsoft Word (you
need to have at least MS 97 for the hyperlinks to work properly). There
should be no problems with the download and conversion to other word
processing programs.

If you are converting the forms to WordPerfect, be
sure to suppress the footer on all applicable documents, beginning on
page two, to avoid it showing up on every subsequent odd page. The
footer contains the PI's signature line and HRPO approval information
that should only be on the first page of each form.

If you are having trouble downloading any of our
forms please try downloading them from Internet Explorer.
